July Online Program Registration

This is the Registration for the Online Group Classes and Individual Lesson For the dates July 1-July 31, 2020.
You must be 18 years of age or older to complete this registration. Students that are 17 years of age and younger can register under the direct guidance and permission of their parents or legal guardians.
OPTION A:
Online Group Class Schedule
Mondays-Fridays 4pm-4:45pm
Price: $40/month
Terms for Online Group Classes:
The price of $40 is whether you attend 1 or all of the Online Group Classes.
There are no refunds.
*In order for advancement in rank an individual lesson needs to be scheduled. This does not guarantee an advancement in rank, but does allow the student and Instructor to have feedback of where they are in terms of their progress and can enhance the focus of their group lessons.
You are also not under any continuing monthly obligation or annual contract.
The Instructor commits to providing online lessons at 4:00-4:45pm Monday-Friday from July 1- July 31. Adjustments to the schedule or credits will be calculated if the instructor is unable to provide a sufficient amount of Online Group Classes with the average amount of totaling no less than 2 classes per week during the month.
Payment can be made via secure ACH bank transfer through an invoice that will be emailed to you upon the processing of your registration or upon the attendance of your first class. If you would rather send a check by mail, please mail the payments to

INDIVIDUAL ONLINE LESSONS
OPTION B
1 Individual Online Lesson (30 minutes) - $60 (required for advancement in rank)
OPTION C
1 Individual Online Lesson (30 minutes) plus the Online Group Classes - $90 (save $10)
OPTION D
2 Individual Online Lessons (30 minutes each lesson) plus the Online Group Classes - $120 (save $40)
OPTION E
4 Individual Online Lessons (30 minutes each lesson) plus the Online Group Classes - $200 (save $80)
Terms: The Online Individual Lessons need to be scheduled within the month of purchase. Once your registration is processed, you will be sent a link to schedule your lessons online. If you prefer to have Dr. DeMassa schedule your lesson you may also schedule via email drd@neuron-do.com or phone 203-866-6000 (text or voicemail).
Payments: Upon the processing of your registration, you will be emailed an invoice from which you can pay via a ACH bank transfer or your can mail a payment to:
Cancellations: You have the ability to cancel and reschedule your lesson online through the link that will be provided. You can also contact the instructor via email drd@neuron-do.com or phone 203-866-6000 (text or voicemail). There is no cancellation fee at this time as you are able reschedule, but they must be rescheduled within the month you purchased the lesson(s). We will make the best effort to schedule your lessons. In the event that the Instructor cancels or is unable to schedule your Individual Online Lesson(s) within the month, they will be credited the next month's lessons.
